ADSS Cymru, along with the Welsh Local Government Association, Solace Wales and Social Care Wales, has published a new paper entitled A Vision for Social Care in Wales.
The paper focuses on the long-term vision of social care for local government. It aims to set out how we can best deliver integrated health and social care systems rooted in local communities, identifying the best elements of both systems and developing more integrated community-based arrangements.
This shift in practice could deliver real benefits in terms of driving true integration and efficients, with a focus on prevention, supporting wellbeing and delivering outcomes.
